Primary Responsibilities:
  • Provide physical therapy services to patient according to physician’s plan of care. This may include, but will not be limited to assessing and evaluating therapeutic / rehabilitative / functional status, participation in the development of the total plan of care and evaluates home environment
  • Initiates physical therapy program and instructs other personnel and / or family caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
  • Prepares and submits clinical and progress summaries in a timely manner
  • Participates in discharge planning for patient
  • Supervises physical therapy assistants according to organization policy and state regulations, treat and re-evaluation patient no less than every 7th day of treatment or 21 days whichever occurs first
  • Maintains contact / communication with other personnel involved in the patient’s care to promote coordinated, efficient care
  • Maintain clinical competency in physical therapy practice and theory
  • Will participate in Agency on-call coverage as outlined in Agency policy

Required Qualifications:

  • Graduate Degree from an Accredited Physical Therapy Program
  • Possess an active and unrestricted Texas Physical Therapist license
  • 2+ years of experience as a Physical Therapist
  • CPR certification or obtain within 30 days of hire
  • Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
  • This position requires Tuberculosis screening as well as proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diptheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor’s diagnosis of disease

Preferred Qualifications:

  • 1 year of Home Health experience
  • Experience with Electronic Medical Records Systems
  • Experience in a home health setting
